LIFE INSURANCE POLICIES DISCUSSED TH EOLB™ AND THE LIFE POLICY GROUP ARE INDIVIDUALLY OWNED BY THE EMPLOYEE, NOT THE EMPLOYER.
OWNERSHIP AND CONTINUATION OF COVERAGE ARE SUBJECT TO THE TERMS OF THE SPECIFIC POLICY ISSUED BY THE INSURANCE CARRIER, INCLUDING BUT NOT LIMITED TO PREMIUM PAYMENTS BEING KEPT CURRENT AND POLICY PROVISIONS.

COVERAGE DOES NOT DEPEND ON CONTINUED EMPLOYMENT; HOWEVER, POLICY BENEFITS, COSTS, AND GUARANTEES ARE GOVERNED SOLELY BY THE CONTRACT.
